Reduce untimely readmissions by transforming your OutPatients into Virtual InPatients
More than a monitoring system, patient@HOME features the Remote Patient Encounter™ where physicians can reach out to their patients via full motion video to take a complete history, collect their vitals, listen to their heart and lung sounds, and even conduct a home sleep study. The application supports a constellation of useful features:
- Front end Compatibility – patient@HOME works with most commercially available wireless sensors – both Bluetooth and Zigby.
- Ad Hoc Conferencing – patient@HOME is a server based system which supports ad hoc conferencing where professionals at multiple Receive Sites can participate in any patient encounter.
- Open Architecture – patient@HOME has been designed under open architecture guidelines where a growing number of third party applications can be integrated as they become available. Also, upon request, Sigma is ready to establish compatibility with in-place hospital information systems.
- Reimbursement – Using patient@HOME, the Receive Site can select and assign the applicable reimbursement codes for each patient encounter.
- Cost per Use – patient@HOME supports cost per use billing, thereby changing a capital expense into an operating expense.
- Central Dispatch – In many cases, patient@HOME Receive sites are widely dispersed – perhaps located even at the homes of the monitoring professionals. The applications offers a calendar function where scheduling and amendments for each encounter can be assigned to available Receive Sites.
- Monitor the Network – The effectiveness of patient@HOME depends on the viability of the underlying network. Accordingly, the application supports ongoing, real time evaluation of the network performance. Using SQL queries, any third party with Administrative access to the network can evaluate
- Technical performance, including response times, dropouts as well as anomalies in the operation of Send and Receive sites,
- Financial performance, including costs and revenues generated per site per day.
- Monitor the Monitors – The patient@HOME network can be used to track and monitor the performance of the monitoring personnel. This surveillance comprises both SQL studies of the performance of the Receive Sites, and the opportunity to communicate directly with the monitors at these sites.
